New York State has changed the grade-levels for which students are required to have a health exam (physical). Beginning with this coming school year, all New York State public school students must have a health exam when they enter school as a new entrant and in Pre-K or Kindergarten and grades 1, 3, 5, 7, 9, and 11.
Please note that a health examination is valid if it took place within the 12-month period prior to the start of the school year in September. Please submit the documentation from your child’s physical as soon as possible if you have not done so already.
New York state has also released a new School Health Examination Form that families and physicians are encouraged to use for the 2018-19 school year. The form will be required beginning with the 2019-20 school year.
